  1. Unexplained Weight Loss

Sudden weight loss without any change in diet or exercise is one of the most overlooked liver cancer symptoms.

  1. Pain or Discomfort in the Upper Right Abdomen

As a tumour grows, it stretches the liver capsule, causing a dull ache or heaviness in the upper right abdomen. Some patients describe it as a feeling of fullness or pressure, not sharp pain.

  1. Swollen Abdomen (Ascites)

If the abdomen looks bloated or distended, especially in a patient with known liver disease, it may be ascites.

That combination of a growing belly with poor appetite is a serious signal.

  1. Jaundice

Yellowing of the skin and whites of the eyes is called jaundice.

  1. Extreme Fatigue That Doesn’t Go Away

Chronic fatigue in a patient with liver disease is often a sign that liver function is declining.

Liver cancer affects metabolism and digestive function. Patients often say they feel full after just a few bites.

Combined with weight loss, this is a strong warning sign.

  1. Nausea and Vomiting

  1. White or Chalky Stools – Dark Urine

These two go together and both point to a disruption in bile flow.

  • Dark, tea-coloured urine means excess bilirubin is being excreted through the kidneys
  • Pale or clay-coloured stools mean bile is not reaching the intestine

Together, they strongly suggest a bile duct blockage, which can be caused by a liver tumour pressing on the duct.

  1. An Obvious Lump Under the Ribs

Any new lump in the abdominal area in a liver disease patient should be taken seriously and investigated immediately with an ultrasound.

This one is for families of patients already diagnosed with cirrhosis or chronic hepatitis.

If symptoms that were previously stable suddenly worsen, more ascites, more confusion (hepatic encephalopathy), more jaundice, more fatigue it may signal that liver cancer has developed on top of the existing disease.

Why Liver Cancer Symptoms Appear Late

The liver is a remarkably resilient organ.

It can function with up to 70% of its tissue damaged without producing obvious symptoms.

This silent nature is what makes liver cancer so dangerous.

By the time pain, jaundice, or weight loss appears the tumour has often already grown large or spread.

This is exactly why screening matters more than waiting for symptoms.

How Is Liver Cancer Diagnosed?

Symptoms alone don’t confirm liver cancer. Diagnosis requires:

  • Ultrasound – first-line screening tool
  • AFP (Alpha-fetoprotein) blood test – a tumour marker elevated in many HCC cases
  • CT scan or MRI – for detailed tumour assessment
  • Liver biopsy – in select cases where imaging is inconclusive

For patients with cirrhosis or chronic hepatitis B – a liver ultrasound every 6 months with AFP monitoring is the recommended surveillance protocol.

Final Word

Liver cancer is serious. But it is not always a death sentence if caught early.

The problem is most patients come in at an advanced stage simply because the early symptoms were misread.
